Your information is very important to us. Please assist us in gathering as much information as possible. Help identify who is engaging in misconduct by including names, titles, addresses, or any other identifying information, relevant to your complaint; what MTA agency is involved; any MTA vendors or contractors involved (if appropriate), what wrongdoing occurred, when, where, for how long, and if there are any witnesses that we may contact. Please be as detailed as possible and include any documents, photographs, or other evidence you may have.
If you provide us with your contact information, we can reach out to you if we have additional questions. You are not required to provide your identity to the OIG. However, you are encouraged to identify yourself and have a way for us to contact you to help the OIG evaluate and investigate the complaints.
